After awaken from 9 years coma, Ishmael that stated "the one who took care big boss during his coma" finally helping him escape from hospital massacre. XOF unit assumed behind the act of hospital massacre. They hunts big boss for....unclear reason. Big boss can barely walk but crawl. His vision hallucinates a lot of mind fuck things around him due the 9 years shallow sleep impact.
